Visitor Waiver

CLOUD FOREST MARTIAL ARTS INSTITUTE — ONE-DAY VISITOR WAIVER, RELEASE, AND ASSUMPTION OF RISK

I acknowledge that martial arts training carries inherent risks of physical injury. I agree to follow all safety instructions from instructors at Cloud Forest Martial Arts Institute and assume responsibility for my own well-being (or my child's well-being) during my visit.

I release Cloud Forest Martial Arts Institute, Greensboro Martial Arts Academy LLC, its instructors, contractors, and staff from any liability for injuries sustained during my visit, except in cases of gross negligence.

I confirm that any medical information I have provided is accurate, and I will inform staff of any conditions that may affect safe training. For a minor visitor, I confirm I am the parent or legal guardian and have authority to sign on the student's behalf.

I understand that Cloud Forest is a Private Membership Organization. This waiver authorizes a single one-day guest visit only and does not grant club membership. Membership requires separate enrollment and the annual club fee.
